Your Registered Practitioner, Kelly Griffiths, is a member of the Institute of Professional Colon Hydrotherapy (IPCH, ipch.org.uk) and the British Wheel of Yoga (BWY, bwy.org.uk). She is insured by Balens, Ltd.
All your data will be stored in line with the 2018 Data Protection Act as a result of the new Data Protection Legislation which came into force in the UK on 25th May 2018, the General Data Protection Regulation (GDPR). Personal data has been defined by the Act as ‘any information relating to an identifiable person who can be directly or indirectly identified’, such as name and contact details.
DATA COLLECTED & WHAT WILL HAPPEN TO IT
The personal data collected about you will include data relating to your name, address, date of birth, wider contact details and data relating to ‘health’. Your practitioner will process this personal data in order to provide you with the best services. Your practitioner may also use your data to communicate with you via email in relation to your present, past or prospective treatments or, if you agree to be on the Trinity | Wellbeing internal mailing list, with promotional information relating to Trinity | Wellbeing.
You will be able opt out from receiving such communications services at any time by emailing your Registered Practitioner, Kelly Griffiths at firstname.lastname@example.org.
Your data will only be used for the purpose for which it was collected.
Your personal information will not be shared with third parties unless you have given your permission or unless your Practitioner is required to disclose the information by law.
Your personal data will be kept in physical (paper) form, secure under lock and key.
Your records will be kept for 7 years following the date of the last class or appointment. In the case of minors, records will be kept for at 7 years after they reach the age of majority (18), i.e. until they reach age 25.
